Registration for GMF la Cigogne
We cannot guarantee the follow-up of your child at the clinic after your pregnancy given the new government laws which now govern the registration of new patients. However, our dedicated nurses will make sure to offer you an appointment around 2-4 weeks of your newborn's life.
In the event that we cannot provide pediatric follow-up, please register your baby on the GAMF (Québec Family Doctor Finder)
as soon as you receive their health insurance card.
Follow-up for registered children
- Visit between 2 and 4 weeks of life: to book the first appointment contact your doctor's administrative assistant.
- Visit at 2, 4, 6, 9 and 12 months: make an appointment via the portal here
- 18 months visit during vaccination at the CLSC.
- As part of the Agir tôt program, all 18-month-old children in Quebec can now benefit from an enhanced vaccination appointment, to allow parents to speak with a nurse about their child's development and health using the ABCdaire 18 months+ tool.

- To make an appointment at the CLSC for vaccination as well as development monitoring 18 months (duration of one hour)
Thereafter, appointments are made annually or as needed.
During clinic appointments for children under 2 years old, please undress your baby and leave him in a diaper with a small blanket. Prepare his vaccination record. It must be brought to each medical visit.
A person who speaks French or English must be present during the child’s appointment to promote good communication.
